1. Purpose

2. Key Activities
2.1 Monthly Community Health Clinics
- Organize monthly mobile or on-site health clinics in partnership with local healthcare providers.
- Provide basic medical consultations, screenings, and referrals.
- Offer maternal and child health services.
- Conduct immunization awareness and support programs.
2.2 Preventative Health Screenings
- Blood pressure, glucose, and cholesterol testing.
- HIV/AIDS awareness and voluntary counseling support.
- TB screening and referral support.
- BMI and general wellness assessments.
2.3 Wellness & Health Education Programs
- Host health awareness workshops (nutrition, hygiene, mental health, substance abuse prevention).
- Promote sexual and reproductive health education.
- Provide family planning information and referrals.
- Organize community fitness and wellness campaigns.
2.4 Mental Health & Psychosocial Support
- Provide access to counseling services through partnerships.
- Conduct stress management and resilience workshops.
- Establish referral pathways to professional mental health practitioners.
- Facilitate support groups for vulnerable groups (youth, elderly, caregivers).
2.5 Chronic Disease Support Programs
- Support individuals living with chronic conditions (diabetes, hypertension).

- Ensure compliance with health regulations and ethical standards.
- Monitor participation rates and service delivery quality.
- Prepare monthly health service reports and impact assessments.
3.2 Health Service Partners
- Provide qualified healthcare professionals.
- Deliver screenings, consultations, and referrals.
- Maintain professional and ethical standards.
- Submit service statistics for reporting purposes.
3.3 Community Health Coordinators
- Mobilize community members to attend health programs.
- Identify vulnerable households requiring assistance.
- Assist with registration and record-keeping.
- Follow up on referred cases when required.
3.4 Monitoring & Evaluation
- Track number of beneficiaries served monthly.
